Read before signing
- Assumption of risk. Baseball carries an inherent risk of injury that cannot be eliminated. Risks include being struck by a thrown or batted ball or a bat, collisions between players, sliding, falls, contact with fences, bases, or equipment, heat, cold, smoke, poor air quality, playing under lights, and travel to and from the field. I understand these risks and I voluntarily accept them on behalf of the player named on my line below.
- Release and waiver. In consideration for the player being permitted to participate, I release and agree to hold harmless the North State League, its organizers, coaches, managers, volunteers, umpires, sponsors, and the owners and operators of any field used, from any claim, demand, or cause of action arising out of the player's participation, including claims based on ordinary negligence. This release does not apply to gross negligence, recklessness, or willful misconduct, which cannot be released under California law.
- Indemnification. I agree to indemnify and defend those released above against any claim brought by me, by the player, or by anyone acting on the player's behalf, arising out of the player's participation.
- Medical treatment. If the player is injured and I cannot be reached, I authorize league organizers or coaches to seek emergency medical care. I am responsible for all resulting medical costs. The league does not provide medical or accident insurance. The player is covered by my own health insurance or by no insurance at all, and I accept that.
- Concussion and head injury. Any player who shows signs of a concussion will be removed from play immediately and may not return that night. Return to play requires written clearance from a licensed health care provider. I have been given the opportunity to review concussion information and I agree to this protocol.
- Age and eligibility. I certify the date of birth I have written below is accurate. Age divisions are based on a player's age as of March 1, 2026. False information makes the player ineligible and may cause the team's games to be forfeited.
- Conduct. I have reviewed the league rules at nsbaseball.com. The player and I will follow them and the direction of the coaches and umpires. Abusive conduct toward umpires, coaches, players, or spectators may result in removal from the park.
- Photographs. I permit photographs and video taken at league games to be used by the league. To decline, write "NO PHOTO" beside my signature.
- Agreement. This document is governed by the laws of the State of California. If any part of it is held unenforceable, the rest remains in effect.
By signing below I state that I am the parent or legal guardian of the player named on that line, that I have read and understood this document, and that I am signing it freely.
